Abstract

Back pain is one of the most common musculoskeletal complaints experienced by women in the third trimester of pregnancy due to physiological, hormonal, and biomechanical changes. One non-pharmacological intervention to reduce back pain is endorphin massage. Coconut oil is commonly used as a massage medium to improve comfort, while the addition of aromatic ginger (Kaempferia galanga L.) is expected to provide analgesic and anti-inflammatory effects through its essential oil content. This study aimed to compare the effectiveness of endorphin massage using coconut oil with endorphin massage using a mixture of coconut oil and aromatic ginger on back pain intensity among third-trimester pregnant women. This study employed a quasi-experimental design with a two-group pretest-posttest design. The study was conducted in the working area of Cibeureum Community Health Center, Tasikmalaya City, from March to May 2026. The sample consisted of 32 third-trimester pregnant women selected using a purposive sampling technique and divided into two groups of 16 participants each. Back pain intensity was measured using the Numeric Rating Scale (NRS) before and after the intervention. Data were analyzed using the Wilcoxon Signed-Rank Test to assess changes within each group and the Mann-Whitney U Test to compare the effectiveness between the two intervention groups. The results showed that the mean back pain score in the coconut oil group decreased from 6.31 to 1.19, with a Wilcoxon test result of p = 0.000 (p < 0.05). In the coconut oil and aromatic ginger group, the mean pain score decreased from 6.25 to 1.38, with p = 0.000 (p < 0.05). The Mann-Whitney U Test showed a p-value of 0.741 (p > 0.05), indicating no statistically significant difference in effectiveness between the two intervention groups. In conclusion, both endorphin massage using coconut oil and endorphin massage using a mixture of coconut oil and aromatic ginger were effective in reducing back pain intensity among third-trimester pregnant women. However, there was no statistically significant difference in effectiveness between the two massage media.
