Abstract
Based on data from Private Hospital X in Majalengka Regency for the period January–December 2025, 424 primigravida women underwent labor induction, and 90 cases (21.2%) experienced induction failure that resulted in cesarean section. The high rate of induction failure highlights the need to analyze the factors associated with this occurrence. This study aims to analyze the association between maternal age, Bishop score, and Premature Rupture of membranes (PROM) with the occurrence of labor induction failure. This study employed a quantitative observational method using a retrospective case-control design. The study sample consisted of 249 participants, comprising 83 cases and 166 controls, selected using purposive sampling. Data analysis was performed using the chi-square test and binary logistic regression. The results of the study showed that maternal age was not significantly associated with the incidence of labor induction failure (p=0.258; OR=1.819). The Bishop score was significantly associated with the incidence of labor induction failure (p<0.001; OR=102.543), as was the PROM (p=0.017; OR=1.907). Multivariate analysis results showed that the Bishop score was a factor significantly associated with the incidence of labor induction failure (AOR = 103.400; 95% CI: 24.305–439.898; p < 0.001), whereas PROM was not significantly associated after being controlled for in the model (p = 0.063). The Nagelkerke R-squared value of 0.571 indicates that the model explains 57.1% of the variation in the incidence of labor induction failure. The conclusion of this study is that cervical ripening, as assessed by the Bishop score, is the most dominant factor associated with labor induction failure in primigravida women undergoing labor induction with misoprostol.
