Abstract
Background: Breast engorgement is a common lactation problem experienced by postpartum mothers due to milk accumulation and vascular congestion within the breast tissue. This condition is characterized by swollen, firm, tense, and painful breasts, which may interfere with the breastfeeding process. One non-pharmacological intervention that can be used to manage this condition is the application of cabbage leaf compresses (Brassica oleracea varietas capitata). Objective: To describe the application of cabbage leaf compresses in managing breast engorgement among postpartum mothers at Dr. Soekardjo Regional General Hospital. Methods: This study employed a case study design involving two postpartum mothers experiencing breast engorgement in the Wijaya Kusuma Postpartum Ward of Dr. Soekardjo Regional General Hospital, Tasikmalaya City, from June 2 to June 5, 2026. Cabbage leaf compresses were applied twice daily for 15–20 minutes over three consecutive days. Outcomes were evaluated using the Six-Point Engorgement Scale (SPES) and the Numeric Rating Scale (NRS). Results: Following three days of cabbage leaf compress application, both patients demonstrated reductions in breast engorgement and pain levels. In Patient 1, the SPES score decreased from 5 to 1, while the NRS score decreased from 3 to 0. In Patient 2, the SPES score decreased from 4 to 1, and the NRS score decreased from 2 to 0. Both patients reported increased breast comfort, reduced swelling and pain, and improved breastfeeding experiences. Conclusion: The application of cabbage leaf compresses (Brassica oleracea varietas capitata) may help reduce breast engorgement in postpartum mothers, as indicated by decreased SPES and NRS scores and improved breastfeeding comfort. This intervention may be considered a safe, simple, and cost-effective non-pharmacological therapy in maternity nursing care. Keywords: breast engorgement, postpartum mothers, cabbage leaf compresses
