Abstract

Latar Belakang: Hipertensi pada lansia dipicu oleh penurunan elastisitas pembuluh darah. Selain terapi farmakologis, penatalaksanaan non-farmakologis seperti isometric handgrip exercise diperlukan untuk membantu mengontrol tekanan darah. Tujuan: Menggambarkan kondisi awal, pelaksanaan tindakan, respon subyek, serta analisis kesenjangan pada lansia hipertensi Ny.M (64 tahun) dan Ny.K (63 tahun) sebelum dan setelah penerapan isometric handgrip exercise. Metode: Karya tulis ilmiah ini menggunakan desain kualitatif dengan pendekatan studi kasus. Implementasi isometric handgrip exercise dilakukan selama lima hari berturut-turut, sekali sehari dengan total durasi 7,5 menit per sesi menggunakan alat handgrip di Desa Cangkoak dan Sindangmekar wilayah Puskesmas Sindangjawa Kecamatan Dukupuntang. Data dikumpulkan melalui wawancara, observasi, dan dokumentasi. Hasil: Kedua subyek merupakan penderita hipertensi menahun yang patuh minum obat Amlodipine namun minim pengetahuan non-farmakologis. Setelah intervensi sesuai standar operasional, kedua subyek menunjukkan respon positif berupa penurunan tekanan darah yang signifikan. Tekanan darah Ny.M turun dari 150/89 mmHg menjadi 125/80 mmHg, dan Ny.K turun dari 160/90 mmHg menjadi 132/79 mmHg. Analisis kesenjangan menunjukkan penurunan tekanan darah Ny.K lebih konsisten karena manajemen stres yang baik dan adanya aktivitas fisik (merotan), sedangkan Ny.M mengalami fluktuasi akibat kurangnya aktivitas fisik harian dan kendala mengelola stres yang memicu gangguan tidur. Kesimpulan: kondisi kedua klien mengalami perubahan tekanan darah, Implementasi isometric handgrip exercise efektif menurunkan tekanan darah, respon kedua klien tekanan darahnya turun, adanya perbedaan konsistensi penurunan tekanan darah Saran: Klien diharapkan mengombinasikan latihan ini dengan aktivitas fisik mandiri ringan secara rutin, dan perlunya dukungan keluarga. Background: Hypertension in the elderly is triggered by decreased blood vessel elasticity. In addition to pharmacological therapy, non-pharmacological management such as isometric handgrip exercise is needed to help control blood pressure. Objective: To describe the initial conditions, implementation of actions, subject responses, and gap analysis in elderly hypertensive Mrs. M (64 years old) and Mrs. K (63 years old) before and after the implementation of isometric handgrip exercise. Method: This scientific paper uses a qualitative design with a case study approach. The implementation of isometric handgrip exercise was carried out for five consecutive days, once a day with a total duration of 7.5 minutes per session using a handgrip device in Cangkoak and Sindangmekar Villages, Sindangjawa Community Health Center, Dukupuntang District. Data were collected through interviews, observations, and documentation. Results: Both subjects were chronic hypertension sufferers who were compliant in taking Amlodipine medication but had minimal non-pharmacological knowledge. After the intervention according to operational standards, both subjects showed a positive response in the form of a significant decrease in blood pressure. Mrs. M's blood pressure decreased from 150/89 mmHg to 125/80 mmHg, and Mrs. K's decreased from 160/90 mmHg to 132/79 mmHg. Gap analysis showed that Mrs. K's blood pressure decrease was more consistent due to good stress management and physical activity (rattling), while Mrs. M experienced fluctuations due to lack of daily physical activity and difficulty managing stress that triggered sleep disorders. Conclusion: both clients experienced changes in blood pressure, Implementation of isometric handgrip exercise effectively reduced blood pressure, both clients' responses decreased blood pressure, there were differences in the consistency of blood pressure reduction Suggestion: Clients are expected to combine this exercise with regular light independent physical activity, and the need for family support.
