Abstract
Type 2 Diabetes Mellitus (T2DM) is a metabolic disease characterized by hypergl-ycemia resulting from impaired insulin secretion, insulin resistance, or a combination of both. Dietary management is one of the main pillars in controlling T2DM, helping to regulate blood glucose levels and prevent complications. This study aimed to determine the implementation of dietary management for Type 2 Diabetes Mellitus and its effect on macronutrient and fiber intake among hospitalized patients at Permata Cirebon Hospital. This study employed a case study design involving two patients with Type 2 Diabetes Mellitus who were hospitalized. Data were collected from May 13–16, 2026, through observation, interviews, 3×24-hour food recalls, food weighing, and assessments of anthropometric, biochemical, physical/clinical, and personal history data. The data were analyzed descriptively based on the Nutrition Care Process (NCP), which includes nutrition assessment, nutrition diagnosis, nutrition intervention, and nutrition monitoring and evaluation. The results showed that both respondents received dietary management in accordance with the stages of the Nutrition Care Process. Protein and fat intake improved during the intervention period, although the first respondent experienced a decrease in protein intake on the third day due to dietary adjustments related to elevated uric acid levels. Carbohydrate intake in both respondents fluctuated and had not optimally met nutritional requirements, which may affect blood glucose control. Fiber intake also did not reach the recommended levels due to suboptimal food consumption and limited intake of high-fiber food sources. Nutrition education was provided to both respondents as part of the nutrition intervention to improve their understanding of and adherence to the diabetic diet. It can be concluded that dietary management of Type 2 Diabetes Mellitus during hospitalization was able to improve some aspects of macronutrient intake, particularly protein and fat intake. However, carbohydrate and fiber intake still require greater attention to meet recommended requirements and support optimal blood glucose control.
