Abstract
ABSTRAK Latar Belakang: Program Jaminan Kesehatan Nasional (JKN) berperan penting dalam meningkatkan akses pelayanan kesehatan, namun kelancaran pelaksanaannya sangat bergantung pada proses klaim yang tepat waktu dan akurat. Pending klaim yang terjadi secara berulang dapat berdampak pada arus kas rumah sakit, pengadaan obat dan alat kesehatan, pembayaran tenaga kesehatan, serta keberlangsungan mutu pelayanan. Temuan ini menegaskan bahwa pending klaim tidak disebabkan oleh satu faktor tunggal, melainkan merupakan hasil interaksi berbagai aspek dalam proses pelayanan dan administrasi klaim. Penelitian ini difokuskan untuk menganalisis pada tiga aspek utama, yaitu administrasi, koding, dan medis, yang diduga berkontribusi terhadap terjadinya pending klaim. Hasil penelitian ini diharapkan dapat menjadi dasar bagi rumah sakit dalam menyusun strategi perbaikan yang lebih terarah dan komprehensif guna menurunkan angka pending klaim serta mendukung keberlanjutan pelayanan kesehatan. Metodologi Penelitian: Penelitian kuantitatif deskriptif dengan pendekatan cross-sectional menggunakan data retrospektif data klaim rawat inap BPJS Kesehatan yang mengalami pending pada tahun 2025 yang dilakukan di RSUD Waled. Hasil: Hasil analisis menunjukkan bahwa pending klaim di RSUD Waled bersifat sistemik dan terjadi secara konsisten setiap bulan, dengan jumlah yang relatif tinggi. Penyebab pending klaim yang paling banyak berasal dari aspek koding sebesar 41,9%, diikuti oleh aspek medis sebesar 35,0% dan aspek administrasi sebesar 23,1%. Pada aspek administrasi, permasalahan yang paling dominan adalah ketidaksesuaian kelas perawatan. Pada aspek medis, penyebab utama adalah kasus asfiksia neonatorum yang tidak memenuhi kriteria Tim Kendali Mutu dan Kendali Biaya (TKMKB). Pada aspek koding, permasalahan yang paling sering ditemukan adalah ketidaktepatan kode diagnosis atau kode yang perlu disesuaikan dengan data klinis berdasarkan hasil verifikasi BPJS. However, the smooth implementation of the program depends heavily on a timely and accurate claims process. Recurrent pending claims can affect hospital cash flow, procurement of medicines and medical devices, payment of healthcare personnel, and the sustainability of service quality. These findings indicate that pending claims are not caused by a single factor but rather result from the interaction of various aspects within healthcare service delivery and claim administration processes. This study focuses on analyzing three main aspects, namely administrative, coding, and medical factors, which are suspected to contribute to pending claims. The results of this study are expected to serve as a basis for hospitals in developing more targeted and comprehensive improvement strategies to reduce the number of pending claims and support the sustainability of healthcare services. Research methodology: Descriptive quantitative design with a cross-sectional approach using retrospective data from BPJS Health inpatient claims that experienced pending status in 2025 at Waled Regional Hospital. Result: The analysis showed that pending claims at Waled Regional Hospital were systemic in nature and occurred consistently every month, with a relatively high number of cases. The most common cause of pending claims originated from the coding aspect (41.9%), followed by the medical aspect (35.0%) and the administrative aspect (23.1%). In the administrative aspect, the most dominant issue was the mismatch of inpatient care class. In the medical aspect, the primary cause was neonatal asphyxia cases that did not meet the criteria established by the Quality and Cost Control Team (TKMKB). In the coding aspect, the most frequent problem was inaccurate diagnosis coding or codes that required adjustment to align with clinical data based on BPJS verification results.
