Abstract
Latar Belakang: Diabetes Melitus (DM) Tipe 2 berisiko menyebabkan komplikasi vaskular, seperti perfusi perifer tidak efektif, yang dapat berujung pada ulkus diabetikum dan amputasi. Tujuan: Menggambarkan implementasi, respons klinis, dan analisis Range Of Motion (ROM) aktif ekstremitas bawah. Metode: Studi deskriptif kualitatif pada dua pasien DM Tipe 2 di RSUD Arjawinangun selama 5 hari. Data dikumpulkan menggunakan observasi perfusi perifer, Monofilament Test 10g, dan pengukuran Ankle Brachial Index (ABI). Hasil: Intervensi ROM aktif (1x sehari, 15 menit selama 5 hari) memperbaiki sirkulasi perifer kedua responden, ditandai dengan CRT <2 detik, akral hangat, dan hilangnya kesemutan. Responden 1 menunjukkan peningkatan sensitivitas kaki yang signifikan (skor awal 1 menjadi 7) dan nilai ABI membaik (kanan 1, kiri 0.92). Responden 2 mengalami pemulihan lebih lambat akibat faktor usia dan komplikasi ginjal. Kesimpulan: ROM aktif terbukti efektif memperbaiki perfusi perifer, sensitivitas kaki, dan nilai ABI pada pasien DM Tipe 2, sehingga direkomendasikan sebagai asuhan keperawatan mandiri. Background: Diabetic ulcers and amputations can result from vascular problems caused by type 2 diabetes melitus (DM), such as inadequate peripheral perfusion. Goal: Describe the application, clini. Method: Over the course of five days, two Type 2 DM patients at RSUD Arjawinangun participated in a qualitative descriptive studycal reaction, and analysis of lower extremity active Range Of Motion (ROM). Ankle Brachial Index (ABI) measurement, the 10g Monofilament Test, and perfusion peripheral observation were used to gather data. Results: Both respondents' peripheral circulation improved with the active ROM intervention (once daily, 15 minutes for 5 days), as evidenced by CRT <2 seconds, warm acral regions, and the elimination of tingling. Respondent 1 demonstrated increased ABI values (right 1, left 0.92) and a notable increase in foot sensitivity (original score from 1 to 7). Due to age and kidney issues, Respondent 2 recovered more slowly. Conclusion: Active ROM is advised as a stand-alone nursing intervention because it has been shown to improve peripheral perfusion, foot sensitivity, and ABI values in Type 2 DM patients.
