Abstract
Latar Belakang: Diabetes melitus merupakan gangguan metabolisme yang ditandai dengan peningkatan kadar glukosa darah dan dapat menimbulkan berbagai komplikasi, salah satunya ulkus diabetik. Ulkus diabetik yang tidak ditangani secara optimal dapat menyebabkan gangguan integritas jaringan, infeksi, hingga amputasi. Setelah tindakan debridement, diperlukan perawatan luka yang tepat untuk mempercepat proses penyembuhan. Salah satu metode yang dapat digunakan adalah modern wound care. Tujuan: Mendeskripsikan implementasi modern wound care pada pasien ulkus diabetik post debridement akibat diabetes melitus di Ruang Imam Bonjol RSUD Arjawinangun. Metode: Penelitian ini menggunakan metode kualitatif dengan pendekatan studi kasus pada dua pasien ulkus diabetik post debridement yang mengalami gangguan integritas jaringan. Hasil: Subjek penelitian terdiri dari Ny. S (55 tahun) dan Tn. B (42 tahun). Setelah dilakukan implementasi modern wound care, kedua pasien menunjukkan perbaikan kondisi luka. Pada Ny. S ditemukan warna dasar luka merah muda, tepi luka mengalami epitelisasi, tidak terdapat slough maupun bau luka. Pada Tn. B ditemukan warna dasar luka merah, tepi luka mengalami epitelisasi, eksudat berkurang, dan jumlah slough menurun meskipun masih terdapat tanda inflamasi. Kesimpulan: Penerapan modern wound care efektif membantu proses penyembuhan luka infeksi pada pasien ulkus diabetik post debridement akibat diabetes melitus. Saran: Penulis berharap perawatan luka dengan penerapan modern wound care dapat dilakukan pada pasien ulkus diabetik post debridement akibat diabetes melitus. Background: Diabetes mellitus is a metabolic disorder characterized by increased blood glucose levels and can cause various complications, one of which is diabetic ulcers. Diabetic ulcers that are not optimally treated can cause impaired tissue integrity, infection, and even amputation. After debridement, proper wound care is needed to accelerate the healing process. One method that can be used is modern wound care. Objective: To describe the implementation of modern wound care in patients with post-debridement diabetic ulcers due to diabetes mellitus in the Imam Bonjol Ward of Arjawinangun Regional Hospital. Method: This study used a qualitative method with a case study approach in two patients with post debridement diabetic ulcers who experienced impaired tissue integrity. Results: The study subjects consisted of Mrs. S (55 years old) and Mr. B (42 years old). After the implementation of modern wound care, both patients showed improvement in wound condition. In Mrs. S, the base color of the wound was pink, the wound edges were epithelialized, there was no slough or wound odor. In Mr. B found a red base wound color, epithelialization of the wound edges, reduced exudate, and decreased slough volume, although signs of inflammation persisted. Conclusion: The application of modern wound care effectively helps the healing process of infected wounds in patients with post-debridement diabetic ulcers due to diabetes mellitus. Recommendation: The author hopes that wound care using modern wound care can be carried out in patients with post-debridement diabetic ulcers due to diabetes mellitus.
