Abstract

Background: Hypertension is a chronic disease that is often accompanied by psychological problems in the form of anxiety due to concerns about health conditions, long-term treatment, and the risk of complications. Anxiety that is not properly managed can worsen physical conditions and reduce patients’ quality of life. The theoretical basis of this study includes the basic concepts of hypertension, covering its definition, etiology, risk factors, pathophysiology, clinical manifestations, complications, diagnostic examinations, and management. In addition, the study discusses the basic concepts of anxiety, including its definition, etiology based on psychoanalytic, interpersonal, and behavioral theories, signs and symptoms, levels of anxiety, risk factors, complications, prognosis, measurement using the Hamilton Anxiety Rating Scale (HARS), as well as pharmacological and non-pharmacological management. This study also describes the concepts of classical music therapy and expressive writing as non-pharmacological nursing interventions to reduce anxiety in patients with hypertension. Objective: To describe nursing care and the implementation of a combination of classical music therapy and expressive writing to reduce anxiety in patients with hypertension. Methods: This study used a qualitative descriptive method with a case study approach involving two patients with hypertension who experienced anxiety, using the nursing care process, including assessment, diagnosis, intervention, implementation, and evaluation. The combination of classical music therapy and expressive writing was administered for four days, while anxiety levels were measured using the HARS instrument. Results: After the intervention, a decrease in anxiety levels was observed in both patients. The HARS score of patient 1 (Mrs. N) decreased from 21 to 7 (66.7%), while patient 2 (Mrs. Y) decreased from 22 to 9 (59.1%). Both patients showed a change in category from moderate anxiety to mild anxiety, accompanied by a reduction in symptoms such as excessive worry, tension, sleep disturbances, and difficulty concentrating. Conclusion: The combination of classical music therapy and expressive writing was effective in helping reduce anxiety levels in patients with hypertension through physiological relaxation and emotional processing mechanisms. Recommendation: This intervention can be considered as one of the non-pharmacological nursing therapies in providing nursing care to patients with hypertension who experience anxiety.
