Abstract
ABSTRAK IMPLEMENTASI INOVASI ICE CHIPS TERHADAP PENURUNAN INTENSITAS RASA HAUS PADA PASIEN CONGESTIVE HEART FAILURE DI RSUD KOTA BANJAR Moch Akbar Dzulfikar¹ Arip Rahman² Yanti Cahyati³ Pembatasan asupan cairan merupakan pilar terapi yang krusial bagi pasien Congestive Heart Failure (CHF) guna mencegah komplikasi fatal akibat kelebihan volume cairan. Namun, intervensi ini kerap memicu keluhan subjektif berupa penderitaan haus yang berat (thirst distress), yang jika tidak dikelola dengan baik dapat menurunkan kepatuhan pasien terhadap program pengobatan. Salah satu intervensi inovasi berbasis bukti untuk mengatasi masalah ini adalah pemberian ice chips. Penelitian ini bertujuan untuk menggambarkan tahapan pelaksanaan proses keperawatan serta menganalisis respon berupa perubahan (penurunan intensitas haus) pada pasien CHF dengan gangguan thirst distress setelah dilakukan tindakan pemberian ice chips. Metode penelitian yang digunakan adalah studi kasus deskriptif dengan pendekatan asuhan keperawatan yang meliputi tahap pengkajian, perumusan diagnosis, perencanaan, implementasi, hingga evaluasi klinis. Subyek penelitian adalah seorang lansia laki-laki berusia 71 tahun (Tn. J) dengan diagnosis medis CHF di Ruang Anyelir RSUD Kota Banjar. Masalah keperawatan utama yang diangkat adalah hipervolemia berhubungan dengan gangguan mekanisme regulasi. Intervensi inovasi yang diberikan berupa ice chips seberat 15 gram yang dilaksanakan selama tiga hari berturut-turut pada 23–25 April 2026. Pengukuran intensitas haus dilakukan sebelum (pre-test) dan 30 menit setelah tindakan (post-test) menggunakan instrumen Thirst Distress Scale (TDS). Hasil pengkajian awal menunjukkan subyek mengalami hipervolemia yang ditandai dengan keluhan sesak napas, edema derajat III pada kedua kaki, tekanan darah 150/90 mmHg, respirasi 30 x/menit, serta keluhan sering merasa haus. Setelah dilakukan implementasi berupa manajemen hipervolemia yang diintegrasikan dengan pemberian ice chips 15 gram, ditemukan adanya respon penurunan intensitas haus yang signifikan pada subyek. Pada hari pertama, skor thirst distress (TDS) subyek sebelum intervensi berada di angka 19 (kategori tinggi), kemudian menurun drastis menjadi 10 setelah diberikan ice chips. Evaluasi selama tiga hari menunjukkan bahwa intervensi ini secara konsisten memberikan efek kenyamanan instan (sesaat) dalam meredakan keluhan haus pasien setiap harinya. However, this intervention often triggers subjective complaints of thirst distress, which, if not managed properly, can reduce patient compliance with treatment programs. One evidence-based innovative intervention to address this problem is the administration of ice chips. This study aims to describe the stages of the nursing process and analyze the response in the form of changes (reduced thirst intensity) in CHF patients with thirst distress after ice chip administration. The research method used is a descriptive case study with a nursing care approach that includes assessment, diagnosis formulation, planning, implementation, and clinical evaluation. The subject of the study was a 71-year-old male (Mr. J) with a medical diagnosis of CHF in the Anyelir Ward of Banjar City Hospital. The main nursing problem raised was hypervolemia related to impaired regulatory mechanisms. The innovative intervention provided was 15 grams of ice chips which were carried out for three consecutive days on April 23–25, 2026. Thirst intensity measurements were carried out before (pre-test) and 30 minutes after the procedure (post-test) using the Thirst Distress Scale (TDS) instrument. The results of the initial assessment showed that the subject experienced hypervolemia characterized by complaints of shortness of breath, grade III edema in both legs, blood pressure of 150/90 mmHg, respiration of 30 x/minute, and complaints of frequent thirst. After the implementation of hypervolemia management integrated with the administration of 15 grams of ice chips, a significant response was found in the subject's thirst intensity reduction. On the first day, the subject's thirst distress (TDS) score before the intervention was at 19 (high category), then decreased drastically to 10 after being given ice chips. A three-day evaluation demonstrated that this intervention consistently provided instant comfort in alleviating patients' daily thirst complaints.
