E-Puskesmas is a Puskesmas management application to manage patient data from registration to patient treatment services. This study aims to determine how the implementation of e-Puskesmas at the Indihiang Puskesmas of Tasikmalaya city in 2025. The implementation of the e-Puskesmas application in terms of Human Resources (HR), organization and technology has not run optimally. One method that can be used to assess an information system is the HOT-Fit model (Human, Organization, Technology and Benefit). A total of 22 Puskesmas in Tasikmalaya City, two of which continue to use e-Puskesmas, while the rest will switch to using Medicy as the Puskesmas Management Information System (SIMPUS). Research Methodology: Descriptive qualitative with case study research type. The subjects were 1 medical records officer, 1 IT staff, 2 poly nurses, 1 midwife, 1 pharmacist and 1 laboratory staff. Data collection was carried out using interview and observation techniques. Data were collected using interview and observation techniques. The research site was located at the Indihiang Health Center, Tasikmalaya city. Results: The evaluation of the implementation of e-Puskesmas shows that the system is easy to use and understand by users, although there is no special training available so understanding is done by self-taught. Organizationally, the use of e-Puskesmas receives support from internal parties and the Health Office. From a technological aspect, the infrastructure is adequate and system problems can be handled well, including solutions to internet network constraints. Overall, e-Puskesmas provides benefits in improving work efficiency and effectiveness, with more advantages than disadvantages.
