Abstract
Continuity of Care (CoC) is a midwifery care approach provided continuously from pregnancy, childbirth, and the newborn period through the postpartum period to monitor the condition of the mother and baby and to detect complications early. The purpose of this care was to provide comprehensive and continuous midwifery care according to the needs of the mother and baby by applying the principles of Woman Centered Care and evidence-based midwifery. The care was provided to Mrs. I, aged 28 years, G1P0A0, at UPTD Puskesmas Pangandaran from April 28 to May 22, 2026, starting from the third trimester of pregnancy at 36 weeks of gestation, through childbirth, the newborn period, and the postpartum period. The methods used included anamnesis, physical examination, observation, supporting examinations, and SOAP documentation. The results showed that Mrs. I's pregnancy progressed physiologically with good maternal and fetal conditions. At 38–39 weeks of gestation, Mrs. I underwent a physiological spontaneous delivery with the application of relaxation techniques, effleurage massage, use of a gym ball, family support, and Early Initiation of Breastfeeding (IMD). The baby was born spontaneously on May 17, 2026, at 10:30 a.m. Western Indonesia Time (WIB), male, weighing 3,200 grams and measuring 51 cm in length, and showed good neonatal adaptation. Newborn care included maintaining body temperature, infection prevention, administration of vitamin K1, HB-0 immunization, breastfeeding support, and education regarding neonatal danger signs. The postpartum period progressed physiologically, characterized by normal uterine involution and lochia according to the postpartum period, adequate breast milk production, and no signs of infection or complications. During the postpartum period, education was provided regarding nutrition, rest, personal hygiene, exclusive breastfeeding, postpartum danger signs, and complementary care in the form of postpartum exercises. Overall, the Continuity of Care provided to Mrs. I was successfully implemented in a comprehensive and continuous manner, thereby supporting optimal maternal and neonatal health.
