Abstract
Midwifery Independent practice Permenkes N0 28 2017 license and implementation of midwife practice is a place for implementation midwifery service activities carried out by individual midwives. Cucu Sutarsih S.ST still use the manual method in provide services to patients. This results obstacles in providing services to patients, so it is necessary to have an Health Information System that can support health services and make it easier for officers to provide services to patients. The purpose design a web based information system to be used by Cucu Sutarsih S.ST. The type qualitative with an information system design prototype. Collection method will be used is interview to 2 informan and service flow observation. Cucu Sutarsih S.ST are still use the manual method, the flow of services in Cucu Sutarsih S.ST includes registration, examination and documentation. The results is a prototype of a web-based health information system. Cucu Sutarsih S.ST not have an SPO on the flow of services, the health information system is designed according to the functional and non-functional needs that exist in the Practice of Midwifery.
