Abstract

INTISARI Latar Belakang: Nyeri persalinan merupakan pengalaman yang hampir universal dialami ibu bersalin, khususnya pada kala I fase aktif. Sekitar 85–90% ibu bersalin di Indonesia mengalami nyeri sedang hingga berat akibat kontraksi uterus dan dilatasi serviks. Nyeri yang tidak tertangani dengan baik dapat memicu kecemasan, peningkatan hormon stres, dan berpotensi memperlambat kemajuan persalinan. Teknik Counterpressure merupakan salah satu intervensi nonfarmakologis yang mudah diterapkan, aman, dan tidak menimbulkan efek samping, namun penerapannya di RS Ciremai Kota Cirebon belum optimal. Tujuan: Penelitian ini bertujuan untuk mengetahui pengaruh teknik Counterpressure terhadap penurunan skor tingkat nyeri persalinan kala I fase aktif pada ibu bersalin di RS Ciremai Kota Cirebon. Metode: Penelitian ini menggunakan pendekatan kuantitatif dengan desain pra-eksperimen (one group pretest-posttest design). Sampel berjumlah 30 ibu bersalin primipara yang berada pada kala I fase aktif, dipilih menggunakan teknik total sampling pada periode Maret 2026. Pengukuran intensitas nyeri dilakukan sebelum dan sesudah pemberian teknik Counterpressure menggunakan instrumen Numeric Rating Scale (NRS). Analisis data dilakukan dengan uji statistik Wilcoxon Signed Rank Test. Hasil: Nilai median tingkat nyeri sebelum intervensi adalah 5,00, yang menunjukkan kategori nyeri sedang. Setelah diberikan teknik Counterpressure, median nyeri menurun menjadi 3,00, yang menunjukkan kategori nyeri ringan. Hasil uji Wilcoxon Signed Rank Test menunjukkan nilai p-value = 0,000 (p < 0,05), sehingga H₀ ditolak. Kesimpulan: Terdapat pengaruh yang signifikan teknik Counterpressure terhadap penurunan skor tingkat nyeri persalinan kala I fase aktif pada ibu bersalin di RS Ciremai Kota Cirebon. Teknik Counterpressure efektif menurunkan intensitas nyeri dari kategori sedang menjadi ringan melalui mekanisme gate control theory dan peningkatan pelepasan endorfin, sehingga layak dijadikan intervensi nonfarmakologis rutin dalam manajemen nyeri persalinan. Kata Kunci: Counterpressure, nyeri persalinan, kala I fase aktif, nonfarmakologis, Numeric Rating Scale ABSTRACT Background: Labor pain is a nearly universal experience among parturient mothers, particularly during the active phase of the first stage of labor. Approximately 85–90% of mothers in Indonesia experience moderate to severe pain caused by uterine contractions and cervical dilation. Unmanaged labor pain can trigger anxiety, elevated stress hormones, and potentially impede labor progress. The Counterpressure technique is a non-pharmacological intervention that is simple, safe, and free of side effects; however, its application at Ciremai Hospital in Cirebon City has not been optimally implemented. Objective: This study aimed to determine the effect of the Counterpressure technique on the reduction of pain scores during the active phase of the first stage of labor among parturient mothers at Ciremai Hospital, Cirebon City. Methods: This study employed a quantitative approach using a pre-experimental design (one group pretest-posttest design). The sample consisted of 30 primiparous mothers in the active phase of the first stage of labor, selected through total sampling during the period of January–March 2026. Pain intensity was measured before and after the application of the Counterpressure technique using the Numeric Rating Scale (NRS). Data were analyzed using the Wilcoxon Signed Rank Test. Results: The median pain score prior to intervention was 5.00 (range 4–6), indicating moderate pain. Following the Counterpressure technique, the median pain score decreased to 3.00 (range 2–4), indicating mild pain. The Wilcoxon Signed Rank Test yielded a p-value of 0.000 (p < 0.05), leading to the rejection of the null hypothesis (H₀). Conclusion: The Counterpressure technique has a statistically significant effect on reducing labor pain scores during the active phase of the first stage of labor at Ciremai Hospital, Cirebon City. The technique effectively reduced pain intensity from moderate to mild through the gate control theory mechanism and the stimulation of endorphin release. Consequently, the Counterpressure technique is recommended as a routine non-pharmacological intervention in labor pain management.
