Abstract
Hypertension is a condition characterized by blood pressure above the normal range, namely systolic blood pressure >140 mmHg and diastolic blood pressure >90 mmHg (PDHI, 2019). According to the World Health Organization (WHO), approximately 1.28 billion adults aged 30–79 years worldwide suffer from hypertension, with the majority living in low- and middle-income countries. High fat and sodium intake are factors that can increase the risk of hypertension and its complications. One of the non-pharmacological therapies recommended to control blood pressure is the Dietary Approaches to Stop Hypertension (DASH) diet, which emphasizes sodium and fat restriction while increasing fruit and vegetable consumption. This study aimed to analyze the implementation of the DASH diet in relation to fat and sodium intake among hypertensive patients in the inpatient ward of Permata Cirebon Regional Hospital. This research employed a descriptive observational case study design. The subjects consisted of two hospitalized hypertensive patients who met the inclusion criteria. Data were collected through interviews, observations, anthropometric measurements, biochemical examinations, physical/clinical assessments, 24-hour food recall, and food weighing methods. The results showed that the implementation of the DASH diet improved the fat and sodium intake patterns of both respondents. Fat intake in respondent 1 increased from 45% at the initial assessment to 90% on the third day, while respondent 2's fat intake decreased from 154% to 97%, with continued adherence to saturated fat restrictions according to DASH diet principles. Sodium intake in both respondents, which initially exceeded the recommended level, decreased during hospitalization and remained below the maximum limit of 1,500 mg/day. Respondent 1's blood pressure decreased from 161/93 mmHg to 120/78 mmHg, while respondent 2's blood pressure decreased from 178/88 mmHg to 167/68 mmHg after three days of DASH diet intervention. This study indicates that DASH diet management can help control fat and sodium intake and contribute to lowering blood pressure in hypertensive patients. Continuous implementation of the DASH diet is recommended as part of nutrition therapy to prevent hypertension-related complications and improve patients' quality of life.
