Abstract
KEMENTRIAN KESEHATAN REPUBLIK INODONESIA POLITEKNIK KESEHATAN KEMENKES TASIKMALAYA PROGRAM STUDI DIPLOMA III KEPERAWATAN CIREBON Karya Tulis Ilmiah, 17 Mei 2026 Implementasi Pemberian Chewing Gum Pada Tn. A dan Ny. M Dengan Masalah Keperawatan Hipervolemia Akibat Gagal Ginjal Kronik di Ruangan Hemodialisa RSUD Arjawinangun Kabupaten Cirebon Muhammad Abiyyu Fawwaz Alghifary1, Komarudin2 , Edi Ruhmadi3 ABSTRAK Latar Belakang: Penderita Gagal Ginjal Kronik (GGK) yang menjalani terapi hemodialisis wajib melakukan pembatasan asupan cairan guna mencegah terjadinya hipervolemia. Pembatasan cairan ini kerap memicu masalah mulut kering dan rasa haus yang hebat, sehingga pasien rentan mengalami peningkatan berat badan interdialitik atau Interdialytic Weight Gain (IDWG). Terapi nonfarmakologis mengunyah permen karet (chewing gum) bebas gula dapat merangsang produksi saliva untuk mengatasi sensasi haus tersebut. Tujuan: Menerapkan implementasi pemberian chewing gum terhadap penurunan rasa haus dan nilai IDWG pada pasien hemodialisis dengan masalah keperawatan hipervolemia. Metode: Penelitian ini menggunakan desain studi kasus deskriptif pada dua subjek, yaitu Tn. A dan Ny. M, di Ruangan Hemodialisa RSUD Arjawinangun. Intervensi dilakukan selama 3 minggu sebanyak 5 kali tindakan menggunakan chewing gum sebanyak 3 butir per sesi. Tingkat rasa haus diukur menggunakan lembar Thirst Distress Scale (TDS) dan pemantauan nilai IDWG. Hasil: Sebelum intervensi, kedua pasien memiliki nilai IDWG kategori berat (Tn. A: 7,69% dan Ny. M: 6,35%). Pasca-intervensi, keluhan rasa haus berkurang, mulut terasa lebih lembab, dan nilai IDWG berhasil turun secara signifikan ke kategori ringan (Tn. A turun menjadi 3,17% dan Ny. M turun menjadi 3,23%). Kesimpulan: Implementasi pemberian chewing gum terbukti efektif dalam merangsang sekresi saliva, mengurangi sensasi rasa haus, serta mengontrol nilai IDWG pada pasien GGK yang menjalani hemodialisis. However, fluid restriction often causes dry mouth and excessive thirst, which may increase the risk of Interdialytic Weight Gain (IDWG). Non-pharmacological therapy using sugar-free chewing gum can stimulate saliva production to help relieve thirst sensation. Objective: To implement the administration of chewing gum in reducing thirst sensation and IDWG values among hemodialysis patients with the nursing problem of hypervolemia. Methods: This study used a descriptive case study design involving two subjects, Mr. A and Mrs. M, in the Hemodialysis Unit of Arjawinangun Regional General Hospital. The intervention was conducted over three weeks with five sessions of chewing gum administration, using three pieces of chewing gum per session. Thirst levels were measured using the Thirst Distress Scale (TDS), along with monitoring of IDWG values. Results: Before the intervention, both patients had severe IDWG values (Mr. A: 7.69% and Mrs. M: 6.35%). After the intervention, thirst complaints decreased, the mouth felt more moist, and IDWG values significantly decreased to the mild category (Mr. A decreased to 3.17% and Mrs. M decreased to 3.23%). Conclusion: The implementation of chewing gum administration was proven effective in stimulating saliva secretion, reducing thirst sensation, and controlling IDWG values in CKD patients undergoing hemodialysis.
