Abstract
Latar Belakang: Diabetes melitus tipe 2 dapat menyebabkan gangguan perfusi perifer akibat hiperglikemia berkepanjangan. Buerger Allen Exercise (BAE) merupakan intervensi nonfarmakologis yang membantu meningkatkan sirkulasi darah perifer melalui perubahan posisi ekstremitas dan kontraksi otot. Penelitian ini menggunakan teori fisiologi sirkulasi perifer dan konsep diabetes melitus tipe 2. Tujuan: Mengetahui implementasi Buerger Allen Exercise pada pasien diabetes melitus tipe 2 dengan masalah perfusi perifer tidak efektif di RSUD Arjawinangun Kabupaten Cirebon. Metode: Penelitian ini menggunakan metode deskriptif kualitatif dengan pendekatan studi kasus pada dua pasien di Ruang Diponegoro RSUD Arjawinangun Kabupaten Cirebon. Penggalian data dilakukan melalui wawancara, observasi, pemeriksaan fisik, pengukuran Ankle Brachial Index (ABI), dan studi dokumentasi. Implementasi dilakukan selama 5 hari berturut-turut. Hasil: Nilai ABI Tn.B meningkat dari 0,83 menjadi 1,08 dan Tn.I dari 0,79 menjadi 1,00, disertai perbaikan suhu akral, pengisian kapiler, dan penurunan parestesia. Kesimpulan: Buerger Allen Exercise efektif meningkatkan perfusi perifer pada pasien diabetes melitus tipe 2. Saran: Buerger Allen Exercise dapat diterapkan sebagai intervensi nonfarmakologis pada pasien diabetes melitus tipe 2 dengan gangguan perfusi perifer. Background: Type 2 diabetes mellitus can cause impaired peripheral perfusion due to prolonged hyperglycemia. Buerger Allen Exercise (BAE) is a non-pharmacological intervention that helps improve peripheral blood circulation through changes in extremity position and muscle contractions. This study used the theory of peripheral circulation physiology and the concept of type 2 diabetes mellitus. Objective: To determine the implementation of Buerger Allen Exercise in patients with type 2 diabetes mellitus experiencing ineffective peripheral perfusion at Arjawinangun Regional General Hospital, Cirebon Regency. Methods: This study used a qualitative descriptive method with a case study approach involving two patients in the Diponegoro Ward of Arjawinangun Regional General Hospital, Cirebon Regency. Data were collected through interviews, observations, physical examinations, Ankle Brachial Index (ABI) measurements, and documentation studies. The intervention was implemented for 5 consecutive days. Results: Mr. B’s ABI value increased from 0.83 to 1.08 and Mr. I’s ABI value increased from 0.79 to 1.00, accompanied by improvements in acral temperature, capillary refill, and decreased paresthesia. Conclusion: Buerger Allen Exercise was effective in improving peripheral perfusion in patients with type 2 diabetes mellitus. Suggestion: Buerger Allen Exercise can be applied as a non-pharmacological intervention for patients with type 2 diabetes mellitus experiencing impaired peripheral perfusion.
