Abstract
Background : LB1 reports at the Health Center are an important source of information for monitoring health services and planning health programs. However, at the Sendang Health Center, report collection is still done manually using Microsoft Excel, so the recording process is slow, prone to errors, and hinders decision making. To overcome this problem, this study aims to design a prototype of a web-based application to speed up, simplify, and improve the accuracy of LB1 report data collection. Research Methodology : This study uses the Research and Development (R&D) method with the waterfall model. Data were collected through interviews and observations at the Sendang Health Center. Application trials were conducted with blackbox testing to evaluate application functionality based on user needs. Research Results : The designed application prototype successfully increased the effectiveness of data collection and reduced recording errors. This application allows officers to upload daily patient visit data and health services directly into the system, which then automatically processes the data and produces more structured and accurate monthly reports. Conclusion : The use of this application increases the effectiveness of recording, accelerates the preparation of reports, and supports data-based decision making at the Sendang Health Center. The application is expected to be an innovative solution in managing LB1 reports. Keywords : Web-based application, monthly reports, Puskesmas, Research and Development, waterfall model.
