S DAN NY. W DENGAN HIPERTENSI DI WILAYAH KERJA PUSKESMAS CIPERNA KABUPATEN CIREBON Ainun Aat Kumala Dewi 1, Ati Siti Rochayati 2, Omay Rohmana 3 ABSTRAK Latar Belakang: Hipertensi merupakan salah satu penyakit tidak menular (PTM) yang prevalensinya terus meningkat, terutama pada kelompok lansia. Kondisi ini dikenal sebagai “silent killer” karena sering tidak menunjukkan gejala namun dapat menimbulkan komplikasi serius seperti stroke, gagal ginjal, dan penyakit jantung. Penanganan hipertensi secara optimal memerlukan pendekatan farmakologis dan non-farmakologis. Salah satu intervensi non-farmakologis yang terbukti efektif, aman, dan mudah diterapkan adalah Isometric Handgrip Exercise (IHE). Latihan ini melibatkan kontraksi otot tangan secara statis tanpa gerakan sendi, bertujuan menurunkan tekanan darah melalui mekanisme peningkatan elastisitas pembuluh darah. Tujuan: karya tulis ilmiah ini adalah untuk mengimplementasikan IHE pada dua lansia yaitu Ny. S dan Ny. W yang menderita hipertensi di wilayah kerja Puskesmas Ciperna, Kabupaten Cirebon, serta menggambarkan perubahan tekanan darah yang terjadi. Metode: studi kasus dengan pendekatan kualitatif. Subjek penelitian adalah dua lansia perempuan berusia di atas 60 tahun yang memiliki tekanan darah ≥140/80 mmHg dan mampu berkomunikasi serta mengikuti latihan IHE selama lima hari berturut-turut. Data dikumpulkan melalui wawancara, observasi, dan dokumentasi, serta dianalisis secara deskriptif kualitatif. Hasil: menunjukkan bahwa setelah dilakukan intervensi IHE selama lima hari, kedua klien mengalami penurunan tekanan darah sistolik dan diastolik yang cukup signifikan. Selain itu, gejala yang dirasakan seperti sakit kepala dan pegal di tengkuk juga berkurang. Hal ini menunjukkan bahwa IHE dapat menjadi salah satu metode alternatif yang sederhana, murah, dan aman untuk membantu mengontrol tekanan darah pada lansia. Kesimpulan: studi kasus ini adalah bahwa IHE merupakan intervensi non-farmakologis yang efektif untuk menurunkan tekanan darah pada lansia dengan hipertensi. Latihan ini direkomendasikan sebagai bagian dari strategi keperawatan komunitas dan dapat dilanjutkan secara mandiri oleh klien dengan dukungan keluarga. It is known as a “silent killer” because it often presents without symptoms but can lead to serious complications such as stroke, kidney failure, and heart disease. Optimal management of hypertension requires both pharmacological and non-pharmacological approaches. One proven effective, safe, and easy-to-perform non-pharmacological intervention is Isometric Handgrip Exercise (IHE). This exercise involves static contraction of hand muscles without joint movement and aims to lower blood pressure by increasing vascular elasticity. Objective: This scientific paper aims to implement IHE for two elderly individuals, Mrs. S and Mrs. W, who suffer from hypertension in the working area of Ciperna Community Health Center, Cirebon Regency, and to describe the resulting changes in their blood pressure. Methods: A case study with a qualitative approach was used. The subjects were two elderly women over the age of 60 with blood pressure ≥140/80 mmHg, who were able to communicate and follow the IHE routine for five consecutive days. Data were collected through interviews, observations, and documentation, and were analyzed descriptively and qualitatively. Results: After five days of IHE intervention, both clients experienced a significant reduction in systolic and diastolic blood pressure. Additionally, symptoms such as headaches and neck pain also decreased. This indicates that IHE can be a simple, low-cost, and safe alternative method to help control blood pressure in the elderly. Conclusion: This case study concludes that IHE is an effective non-pharmacological intervention for reducing blood pressure in elderly individuals with hypertension. It is recommended as part of community nursing strategies and can be continued independently by clients with support from their families.
