Abstract

TINJAUAN KELENGKAPAN PENGISIAN REKAM MEDIS GIGI PADA KASUS NEKROSIS PULPA DI PUSKESMAS CIPEDES TAHUN 2022 54 Halaman, 5 Bab, 5 Gambar, 7 Tabel, 15 Lampiran ABSTRAK Latar Belakang: Panduan rekam medis kedokteran gigi terbagi menjadi empat bagian yaitu identitas pasien, tabel perawatan, odontogram dan lampiran pelengkap/penunjang. Hasil Riskesdas tahun 2018 sebesar 45,3% masyarakat Indonesia mengalami permasalahan gigi rusak/berlubang/sakit, jika dibiarkan menyebabkan nekrosis pulpa. Nekrosis pulpa merupakan kondisi pulpa/jaringan telah mati, tidak adanya pembuluh darah dan tidak berfungsinya syaraf pulpa pada gigi. Pasien yang mengalami nekrosis pulpa di Puskesmas Cipedes sebesar 22% dari total kunjungan poliklinik gigi tahun 2022. Penelitian ini bertujuan untuk mengetahui kelengkapan pengisian rekam medis kasus nekrosis pulpa di Puskesmas Cipedes Tahun 2022 berdasarkan Panduan Rekam Medis Kedokteran Gigi. Metodologi Penelitian: Jenis penelitian kuantitatif deskriptif. Populasi dan sampel penelitian ini adalah rekam medis gigi kasus nekrosis pulpa tahun 2022, dengan sampel 82 rekam medis. Pengambilan sampel menggunakan teknik simple random sampling. Hasil Penelitian: Hasil Penelitian menunjukkan bahwa identitas pasien (73,90%), pelaporan penting (71,41%), autentifikasi (8,17%) dan pencatatan yang baik (66,67%). Kelengkapan terendah yaitu pada komponen autentifikasi sebesar 8,17%. Ketidaklengkapan tersebut dikarenakan kurangnya ketelitian petugas dan tidak dilakukannya pengecekan ulang. Simpulan: Rata-rata kelengkapan pengisian rekam medis gigi yaitu sebesar 55,04%. Petugas diharapkan mengisi rekam medis dengan lengkap guna sebagai aspek hukum dan diharapkan menggunakan formulir yang sesuai dengan panduan rekam medis kedokteran gigi. In 2018, the National Health Survey reported that 45.3% of Indonesians suffered from tooth decay, cavities, or pain, which can lead to pulp necrosis if left untreated. Pulp necrosis is characterized by the death of pulp tissue in a tooth, resulting in non-functioning nerves and a lack of blood vessels. In 2022, pulp necrosis accounted for 22% of dental clinic visits at the Cipedes Community Health Center. This study aims to assess the completeness of medical records for pulp necrosis cases in 2022 at the Cipedes Community Health Center, based on the Dental Medical Recordkeeping Guide. Methodology: This is a descriptive quantitative study that used sample of 82 dental medical records from 2022 pulp necrosis cases, selected through simple random sampling. Research Results: Completeness of patient identity (73.90%), important reporting (71.41%), authentication (8.17%), and good recording (66.67%). Authentication had the lowest completeness due to officer accuracy and rechecking issues. Conclusions: The average completeness of dental records is 55.04%. Completeness is crucial for legal purposes and requires the use of appropriate forms following dental record guidelines.
