Abstract
Continuity of Care (COC) is continuous midwifery care provided from pregnancy, childbirth, postpartum, newborn care, to family planning. This care aimed to provide comprehensive and continuous midwifery care for Mrs. A, 29 years old, G2P1A0, at 39–40 weeks of physiological pregnancy in the working area of Bungursari Health Center, Tasikmalaya City. The method used was a case study with a midwifery management approach and SOAP documentation. The results showed that pregnancy, childbirth, postpartum, and newborn care progressed physiologically without complications. The baby was born spontaneously, cried strongly, and received standard neonatal care. The care continued with postpartum and newborn monitoring, as well as family planning counseling. The implementation of COC supported continuous care and the health of both mother and baby.
