Abstract
Continuity of Care (COC) is a continuous midwifery care provided from pregnancy, childbirth, postpartum, and newborn care to family planning. The purpose of this care was to provide comprehensive and continuous midwifery care for Mrs. A, 29 years old, G2P1A0, at 39–40 weeks of physiological pregnancy in the working area of Bungursari Health Center, Tasikmalaya City. The method used was a case study with a midwifery management approach and SOAP documentation. The results showed that the pregnancy, childbirth, postpartum period, and newborn care progressed physiologically without complications. The baby was born healthy and received Early Initiation of Breastfeeding (IMD), vitamin K, eye ointment, and exclusive breastfeeding. The care also included family planning counseling based on the principle of informed choice. The implementation of COC provided continuous care and supported the health of both mother and baby.
